To register a student you will need ALL of the following

Certified Birth Certificate

Child’s Social Security Card
-
Proof of Fayette County residency:
Current Tax Bill or Closing Statement /
Settlement Statement
~ OR ~
Current Signed Lease Agreement
-
Current Gas or Electric Bill (initiation of utility
service) displaying
customer’s name and address of service
-
*Immunization Record on the Georgia State Form 3032
or Form 3231
(the Varicella Vaccine and Hepatitis B Vaccine are now required for
attendance)
-
*Eye, Ear and Dental Record on the Georgia State
Form 3300
-
Guardianship papers, if the child does not live
with his/her natural parents

Photo ID of Parent/Guardian registering student

Withdrawal Form from previous school

Transcript and/or report card
